APPLICATION GUIDELINES

Deadline: Must be postmarked by March 25, 2024

 

           ENTRY SUBMISSION INSTRUCTIONS

All of the following are required for student registration.

See sidebar at left for required documents.

 

_____ 1. TEACHER ELIGIBILITY FORM

_____ 2. Appropriate documentation to support the teacher activities listed on Teacher Eligibility Form.  

_____ 3. For each student:  TWO completed copies of STUDENT EVALUATION FORM (with repertoire filled in)

_____ 4. For each student:  STUDENT ENTRY FORM  (Note “Student Photo   Permission” section requiring parent signature.)

_____ 5. For each studentDocumentation for student activities.  See STUDENT ELIGIBILITY page for options.  

_____ 6. A  self-addressed 8 ½” x 11” envelope bearing four (4) stamps for the return of competition results/judges’ comments.  

 

_____ 7. ONE CHECK for total of fees (Competition-$45 per student; Festival-$25 per student) payable to “JOINT WEST SUBURBAN ISMTA.

Teachers will be notified of performance times by e-mail.  Repertoire changes will not be allowed. 

 

PLEASE DO NOT SEND ANY MATERIALS WITH ‘TRACKING’, WHICH SLOWS THE RECEIPT PROCESS.

Senior Division

      Grades 10-12 (Repertoire comparable to AiM Levels 10-12)
Junior Division
      Track 1 ~ Grades 6 & 7 (Repertoire comparable to AiM Levels 8-10)
      Track 2 ~ Grades 8 & 9 (Repertoire comparable to AiM Levels 9-11)
Elementary Division
      Track 1 ~ Grade 5 (Repertoire comparable to AiM Levels 4-7)
      Track 2 ~ Grade 5 (Repertoire comparable to AiM Levels 7-9)
